CCPA information request form

Under the California Consumer Privacy Act (CCPA), current California Residents have the right to request that Rewards Network Establishment Services Inc. (RN) disclose the following to you:

1. Categories of personal information RN has collected about you in the past 12 months;
2. Categories of sources from which your personal information is collected;
3. Business or commercial purpose for collecting your personal information;
4. Categories for Third Parties with whom RN shares your personal information; and
5. Specific pieces of personal information RN has collected about you.

Please note that your information is never sold by Rewards Network. We will attempt to provide a response to your request within forty-five (45) days. If we need additional time to respond to your request, we will notify you of such an extension and the time period needed to respond.
